Swinerton Builders is seeking qualified subcontractors & suppliers to provide pricing (bids) on the OC Sanitation District Headquarters Complex - located at Fountain Valley, CA. Construction of a new Administration Building that is three levels, approximate 108,000 SF office building that is Type IIIB construction consisting of mass timber and steel-framed structure. Work as indicated in Plant 1. New Pedestrian Bridge connecting the new building to secure the perimeter of Plant 1. Various other Work as indicated in the contract documents.
A. Two (2) weeks prior to bid (by March 15, 2021) any Subcontractor with bids over $450,000 or anticipated work within the first 90 days shall provide:
1. Completed Workers’ Compensation Experience Rating Form (also referred to as an Experience Rating Worksheet) and completed and signed copies of their CAL/OSHA 300/300A Logs and Summaries for each of the last three (3) completed years.
2. Completed and signed copy of the “Safety Declaration of Contractor (5 pages form BF-6) and all required backup.
B. If bidder has an accident frequency factor below 1.00 or a workers’ compensation experience modification rate issued and published by the Workers’ Compensation Insurance Rating Bureau of California (WCIRB) below 1.00 the bidder is good to bid.
C. If bidder has any Serious, Willful, Repeat, Serious and Willful, or Failure to Abate citations pursuant to Part 1 of Division 5 of the Labor Code and has not taken appropriate corrective action to abate further such violations; or if the Bidders has an accident frequency factor between 1.01 and 1.25 and has not taken the appropriate corrective action to reduce its accident frequency experience; or if the Bidder has a workers’ compensation experience modification rate issued and published by the Workers’ Compensation Insurance Rating Bureau of California (WCIRB) between 1.01 and 1.25 and has not taken appropriate corrective action to reduce its workplace injuries, illnesses, and workers’ compensation losses, or if the Bidder does not have injury prevention programs instituted to the extent required by Section 3201.5 or 6401.7 of the Labor Code, the bidder cannot bid.
D. If bidder has an accident frequency factor is above 1.25 or a workers’ compensation experience modification rate issued and published by the Workers’ Compensation Insurance Rating Bureau of California (WCIRB) above 1.25 the bidder is rejected and cannot bid.
